Each year, parents must sign and submit health forms to the Nurse's Office. These forms are submitted through Magnus Health, which can be accessed from the parent resource board in our Blackbaud Portal.

Feel free to download and complete the following forms as needed.

Please note: the below forms might not represent all required forms for your child. Please log in to Magnus Health to complete all required forms.

• The Physician OTC Form is needed for each school year to give the Nurse an order to administer the over-the-counter medications listed on the form. It must be signed by a Physician or Nurse Practitioner.
• The Allergy Action Plan and Asthma Action Plan are for students that have inhalers or epipens they carry or leave at the Nurse’s Office.
• The EL2, EL3, and GA4 Forms are for students in grades 6-12 who participate in sports and must be completed every school year. The athletic department handles those forms.
• The Florida School Health Entry Form is for New Students enrolling at CSN for the first time.

CSN assigns summer homework to all students entering Kindergarten through Grade 12.

Summer homework provides a valuable opportunity for students to review concepts learned during the school year and helps to prevent loss of academic progress. Consistent reinforcement of skills is crucial to academic success, and summer homework allows students to maintain their progress and be better prepared for the upcoming school year.
